Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival.
Payment Options
We accept all Major credit cards: Visa, MasterCard, American Express and Discover.
We offer interest free financing with our finance partner CareCredit.
Dental Insurance
We will do all that we can to maximize your dental insurance benefits and minimize your out of pocket expenses. We are premier and PPO providers for Delta Dental and accept several major insurance companies.
As a service we file insurance claims for you, which are submitted promptly after treatment is rendered and we make every effort to receive payment on your behalf.
However if your insurance company has not paid your account in full within 60 days, the balance of the account will be your responsibility. Your insurance policy is a contract between you and your insurance company; we are not a party to that contract. Please be aware that some, and perhaps all of the services provided may be non-covered services. We will continue to assist you in any way that we can to receive payment and we are always available to answer your questions.
